Q- What do I need to bring to my visit?
A- You will need to bring valid California ID. You will need to have a condition that qualifies, and any Medical paperwork to support your diagnosis. Find out more.
The fee for the evaluation is $150. We accept cash and credit (Visa, MC and Discover.) Sorry no checks.
We recommend that you get pre-qualified for free and bring your certificate with you to your appointment.
